Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by creating and implementing tailored treatment plans and providing therapeutic interventions in home and community settings.

The role emphasizes collaboration with a treatment team, adherence to regulatory standards, thorough documentation, and active referral development to maintain a full caseload. A bachelor’s degree and at least two years of relevant experience are required.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team.
• Provide individual, group, and family therapeutic interventions as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of all client interactions, treatment plans, and progress notes in compliance with standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development.

Education
