Job summary

The Family Tree, Inc. is seeking a Family Support Specialist to provide strength-based in-home family services for children prenatal-3.

You will partner with families to identify needs, develop action plans, and mobilize resources, maintaining a caseload of 15-25 families. Responsibilities include completing intakes and goal plans within 30 days, conducting regular progress reviews, and collaborating with the clinical supervisor.

Qualifications

• Bachelor’s degree in Behavioral Sciences is preferred.
• 3–5 years’ experience in Human or Social Services.
• At least 1 year working with children and families; case management/coordination preferred.
• Experience with diverse populations.
• Willingness to work flexible hours, including evenings/weekends.

Responsibilities

• Complete family intake and assessment for each family within 30 days.
• Complete family goal plan within 30 days for each family.
• Review plans at least every 30 days to measure progress.
• Role model effective parenting and nurturing skills.
• Provide direct teaching using curriculum to build healthy parent/child relationships.
• Identify and mobilize community resources and link families to services.
• Maintain accurate case files for each family.
• Meet weekly with Clinical Supervisor to discuss cases.
• Complete all required documentation and reports on time.
• Attend staff meetings and agency trainings.
• Participate in agency work teams as assigned.
