Home Healthcare, Hospice and Community Services is seeking a Family Support Specialist to provide strengths-based, trauma-informed support to parents, caregivers, children, and kinship families in the community.

The role includes home visits, case management, resource navigation, parent education, advocacy, and collaboration with schools, healthcare providers, and social service agencies to help families achieve stability and growth.

Qualifications

• Experience providing services to families in need.
• Ability to work with diverse populations and communities.
• Strong communication and interpersonal skills.
• Knowledge of local community resources and systems.

Responsabilités

• Conduct home visits and provide family support services per program standards.
• Develop individualized, family-centered goals and service plans.
• Maintain caseload and coordinate referrals, follow-up, and ongoing support.
• Connect families with healthcare, housing, education, childcare, and financial resources.
• Support grandparents, relatives, and kinship caregivers.
• Promote positive parenting and child development; ensure family well-being.
