Job summary

Centerstone is seeking a field-based Family Support Specialist to provide in-home visitation and linkage to services in Cannon County, TN. The role emphasizes building trusting relationships, strengthening parent-child dynamics, and enhancing family well-being through the Family Service Plan.

Responsibilities include home visits, screenings, referrals, record-keeping, and participation in required trainings. A bachelor’s degree in a related field is required along with relevant experience.

Qualifications

• Bachelor’s degree in psychology, social work or related field required.
• Two years of professional experience with youth and families preferred.
• Infant mental health endorsement preferred.

Responsibilities

• Provide in-home visitation and family support using evidence-based practices.
• Build trusting relationships and assist with goal setting on the Family Service Plan.
• Administer screenings and refer families to health and community services.
• Document encounters, track progress, and complete required reports.
• Attend mandatory trainings and meetings as required.
