Job summary

Centerstone seeks an in-home visitation specialist to provide linkage and support to families in Van Buren County, with visits typically lasting about an hour in the family’s home. The role supports healthy child development and resilience through structured Family Service Plans and referrals.

Occasional evenings or weekends, travel within the county, and mandatory attendance at the Tennessee State Summit are required.

Qualifications

• Bachelor’s degree in psychology, social work, or other behavioral health care field is required.
• Two years of professional experience working directly with youth and families is preferred.
• Requires valid driver’s license in good standing.

Responsibilities

• Provide in-home visitation services and linkage to supporting services.
• Build trusting relationships and support family functioning.
• Administer appropriate screening tools and referrals as needed.
• Maintain Family Service Plans and track progress over time.
• Prepare monthly, quarterly, and annual reports.
