Job summary

Health Connect America is hiring for Family Centered Treatment professionals to deliver in-home and community-based services. You will work closely with families, develop service plans, and coordinate with the treatment team to ensure fidelity and positive outcomes.

The role requires a Bachelor's degree in human services or related field, with experience in youth/family work and FCT certification preferred.

Qualifications

• Current Family Centered Treatment Certification is preferred in all areas.
• Virginia QMHP eligibility may be required; state-specific credentials exist.
• Experience working with youth or families is expected across multiple states.

Responsibilities

• Provide Family Centered Treatment (FCT) services in client homes and communities.
• Complete FCT checkoffs and internal trainings within deadlines.
• Develop and monitor Service Plans with the treatment team and coordinate discharge planning.
• Conduct psychoeducation and family interventions aligned with the FCT model.
• Maintain accurate clinical records and timely documentation.
