Job summary

Health Connect America, Inc. is hiring for Family Centered Treatment professionals in Virginia. You will deliver services to children, adolescents, and families in their homes and communities, coordinating with treatment teams to develop and monitor Service Plans.

The role requires a Bachelor’s degree in human services or related field and at least two years of experience with youth or families, with certification in FCT preferred. Be Well With HCA benefits and supportive scheduling are provided.

Qualifications

• Two years’ experience working with youth or families.
• Current Family Centered Treatment Certification is preferred in all areas.

Responsibilities

• Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
• Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
• Provide 24/7 on-call coverage to a caseload of families and children, with a flexible schedule.
• Develop Service Plans in coordination with Treatment Team and maintain plans per authorization processes.
• Coordinate and oversee assessment activities and engage medical and non-medical providers in the planning process.
• Ensure linkage to clinically appropriate services including psychological and psychiatric evaluations.
