Responsibilities

• Provide intensive, in-home Family Centered Treatment (FCT) services for children, adolescents, and families.
• Deliver in-home and community-based therapy using a structured, phase-based model.
• Coordinate care with schools and other service systems to ensure integrated delivery.
• Maintain accurate documentation and complete required FCT forms.
