Job summary

Health Connect America, Inc. is seeking a Family Centered Treatment Therapist to deliver intensive, in-home services for children and families in Georgia. You will implement phase-based FCT therapy, engage families, and coordinate with schools and other providers to ensure integrated care.

A master's in social work is required; two years’ experience with youth/families is preferred, and FCT certification is a plus. This role emphasizes flexibility, ongoing training, and fidelity to the model.

Qualifications

• Master's Degree in Social Work required.
• Two years' experience working with youth or families preferred.
• Current Family Centered Treatment Certification is preferred.

Responsibilities

• Provide intensive, in-home Family Centered Treatment (FCT) services for children, adolescents and families.
• Develop and implement service plans in collaboration with treatment teams and families.
• Coordinate care with schools, medical providers, child welfare, and other involved systems.
