Job summary

Health Connect America in Georgia is seeking a Family Centered Treatment Therapist to deliver in-home services for children, adolescents, and families using a structured model-driven approach.

You will work closely with treatment teams, families, and community partners, maintaining documentation and pursuing ongoing fidelity training. A master’s degree and two years of relevant experience are required, with FCT certification preferred.

Qualifications

• Two years’ experience working with youth or families.
• Minimum of a Master's Degree in Social Work, Counseling, or related field is required.
• Current Family Centered Treatment Certification is preferred.

Responsibilities

• Provide Family Centered Treatment (FCT) services in alignment with model fidelity requirements and clinical service definitions.
• Deliver in-home and community-based therapy to children, adolescents, and families using a structured, phase-based treatment model.
• Develop, implement, and adjust service plans in collaboration with treatment teams, families, and other involved systems.
• Facilitate family engagement, goal setting, psychoeducation, and skill-building aligned with identified treatment needs.
• Coordinate care with schools, medical providers, child welfare, juvenile justice, and natural supports to ensure integrated service delivery.
• Maintain accurate, timely documentation, complete all required FCT forms, and support discharge planning and aftercare coordination.
