Job summary

Health Connect America is seeking a Family Centered Treatment (FCT) Therapist for Georgia HOPE, delivering intensive in-home services to children, adolescents and families. You will implement structured, model-driven therapy in homes and communities and coordinate with schools and care teams.

Requirements include a Master’s degree and two years’ experience with youth or families; FCT certification is preferred.

Qualifications

• Master's Degree in Social Work, Counseling, or related field required.
• Two years’ experience working with youth or families.
• Current Family Centered Treatment Certification preferred.

Responsibilities

• Provide Family Centered Treatment services in alignment with model fidelity requirements.
• Deliver in-home and community-based therapy to children, adolescents and families.
• Develop and adjust service plans with treatment teams, families, and other systems.
• Facilitate family engagement, goal setting, psychoeducation, and skill-building.
• Coordinate care with schools, medical providers, child welfare and natural supports.
