Job summary

Health Connect America’s Georgia HOPE program is seeking a Family Centered Treatment Therapist to deliver in-home, structured family therapy for children, adolescents, and their families. Responsibilities

• Provide FCT services in alignment with fidelity requirements.
• Deliver in-home and community-based therapy to children, adolescents, and families.
• Develop, implement, and adjust service plans with treatment teams, families, and other involved systems.
• Facilitate family engagement, goal setting, psychoeducation, and skill-building aligned with identified treatment needs.
• Coordinate care with schools, medical providers, child welfare, juvenile justice, and natural supports to ensure integrated service delivery.
• Maintain accurate, timely documentation, complete all required FCT forms, and support discharge planning and aftercare coordination.
