Job summary

Georgia HOPE is seeking an experienced FCT Therapist to deliver intensive in-home clinical services to children, adolescents, and families. You will implement the Family Centered Treatment model with fidelity, coordinating with schools, medical providers, and child welfare to ensure integrated service delivery.

The role requires a Master’s degree in a related field and two years of experience with youth or families.

Qualifications

• Master’s Degree in Social Work, Counseling, or related field is required.
• Two years’ experience working with youth or families.
• Current Family Centered Treatment Certification is preferred.

Responsibilities

• Provide intensive in-home clinical services to support children, adolescents, and families.
• Deliver in-home and community-based therapy using the Family Centered Treatment model.
• Develop and adjust service plans with treatment teams, families, and other involved systems.
• Facilitate family engagement, goal setting, psychoeducation, and skill-building.
• Coordinate care with schools, medical providers, child welfare, and other supports.
• Maintain accurate documentation and support discharge planning and aftercare coordination.
