Job summary

Integrated Services for Behavioral Health (ISBH) in Ohio is seeking a Care Coordinator to lead child and family teams, coordinate services at home and in the community, and ensure voice and choice in care plans. The role supports youth with complex behavioral health needs across OhioRISE, requiring travel and strong documentation skills.

Qualified candidates will have relevant behavioral health experience, a driver’s license, and the ability to bill Medicaid.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector fields.
• Three years with a high school diploma or equivalent; or two years with an associate or bachelor's; or one year with a master's.
• Hi-Fidelity Wraparound experience preferred (certification provided at time of employment).
• Two years in a care coordination role requiring high efficiency and attention to detail preferred.
• Experience with autism spectrum disorders and developmental disabilities preferred.
• Ability to bill Medicaid without restrictions.
• Valid driver's license and ability to operate a motor vehicle.
• Strong verbal and written communication skills.
• Ability to travel within the assigned service area.
• Confidentiality and HIPAA compliance required.

Responsibilities

• Develop, lead, and facilitate team meetings for the young person and their identified team.
• Provide care coordination within the family’s home, community, or preferred location.
• Partner with the youth, family, natural supports, and service providers to identify needs and services.
• Maintain required documentation to access and maintain OhioRISE benefits.
• Attend mandatory trainings and supervision which may require travel.
• Follow ISBH policies and confidentiality requirements.
• Utilize databases and Electronic Health Record systems for client documentation.
• Travel within service area; all other duties as assigned.
