Job summary

The Center for Family Development, based in Columbia, TN, is seeking a case manager to work with children raised by relative caregivers to empower families with tools and resources to keep children out of foster care.

The role includes in-home visits, caregiver support groups, advocacy, and helping families access TANF, SNAP, and other supports, with travel across the region and a full-time schedule.

Qualifications

• BSW or BS degree preferred; Social Work background a plus
• 1–2 years of case-management experience a plus

Responsibilities

• Monthly in-home visits with eligible children within the family home
• Quarterly caregiver support groups
• Family advocacy and crisis intervention
• Assistance with TANF, SNAP, Child Only Grant applications
• Help locating birth certificates, Social Security cards, and insurance cards
• Emergent caregiver financial assistance
• Support for children experiencing trauma due to parental incarceration, death, or substance use
