Job summary

SAFY is seeking a full-time Case Manager for our Columbia division in South Carolina. This role involves traveling daily to provide in-home, community-based services to families and youth, completing referrals and intakes, and coordinating with county agencies and schools to ensure effective interventions for foster youth.

Qualifications

• Bachelor’s degree in social work, counseling, psychology, sociology, or related human services field.
• LSW or LPC license eligible preferred.
• Two years of relevant experience; child welfare or work with children preferred.

Responsibilities

• Travel daily to provide in-home, community-based services to families and youth in caseload.
• Complete referral reviews and intake assignments; communicate service availability to assigned clients.
• Provide instruction, intervention, and assistance as needed through independent judgment or as part of the treatment team to preserve the family unit.
• Coordinate with county agency and attend family sessions to promote successful clinical interventions for foster youth.
• Build and maintain relationships with families and external case managers.
• Coordinate and monitor service provisions for youth and family in the community with schools, courts, and other parties involved.

Education
