Job summary

SAFY is seeking a full-time Case Manager for our Columbia division in South Carolina. You will travel daily to deliver in-home, community-based services to families and youth, conduct assessments, and coordinate with schools, courts, and county agencies to ensure effective interventions.

The role requires a Bachelor’s degree in a related field, 2 years of relevant experience (child welfare preferred), and a valid driver license with travel ability.

Qualifications

• Bachelor’s degree in social work, counseling, psychology, sociology, child and family studies, or related human services field.
• Two years of relevant experience; child welfare preferred.
• LSW, LPC or license eligible preferred; valid driver license and travel ability.

Responsibilities

• Travel daily to provide in-home, community-based services to families and youth in caseload.
• Complete referral reviews and intake assignments; communicate service availability to clients.
• Provide instruction, intervention, and assistance as needed with the treatment team to preserve the family unit.
• Coordinate with county agency and attend family sessions to promote clinical interventions with foster youth.
• Build and maintain relationships with families and external case managers.
• Coordinate and monitor service provisions for youth and family with schools, courts and other parties.
• Develop and ensure completion of assessments including IPTs, PSDs, and Mental Health Service Plans.
• Document outcomes of home visits and safety assessments; maintain confidentiality.
• Work nontraditional hours; participate in on-call schedule and QA processes.
