• Bachelor’s degree in social work or related field with one year of youth and families experience.
• Clear full three-tier background clearance.

Aufgaben

• Attend court hearings, CFTMs, treatment team meetings and other child-related meetings.
• Conduct home visits, face-to-face visits and family visits.
• Develop timely, individualized master service plans with relative caregivers and youth.
• Link and advocate for community resources and formal services; support informal networks.
• Complete required documentation in a timely manner.
• Provide parent training and empower family members.
• Transport children and families as necessary.
