Job summary

The Center for Family Development in Columbia, TN is seeking a dedicated case manager to support children raised by relative caregivers. The role involves in-home visits, advocate work, and assisting families with essential services to keep children out of foster care. Flexible schedule, some evenings, and Saturdays are required.

Full-time position (40 hours/week) with mileage reimbursement. Requires a four-year degree (preferably in social work) and at least 2 years of case management

• Emergent caregiver financial assistance.
• Extensive travel to client homes across the region; heavy case documentation and meetings.

Job description

The Center for Family Development (Columbia, TN home office location) is seeking a case manager to work with children that are being raised by relative caregivers that have custody of child(ren), to include: grandchildren, nieces, nephews, siblings, etc. This position will work closely with caregivers and children within local counties, (within their homes), to empower families with the tools, resources, and support needed to keep children out of the foster care system, build resilience, and overcome barriers to care.

This position will provide guidance to facilitate the following core services:

• Monthly in-home visits with all of the eligible children within the family home
  
• Quarterly caregiver support groups
• Family advocacy and crisis intervention
• Information about applying for TANF, SNAP, Child Only Grant
• Support in locating birth certificates, social security cards, and insurance cards for Families
• Emergent caregiver financial assistance

This position permits a flexible schedule and works occasional evenings and Saturdays.

• $45K/year to start, PLUS mileage reimbursement @ .76 per mile.
• Monday to Friday, 8am-4:30pm position requires a four-year degree, preferably in social work
• Seeking minimum 2 years case management experience
• Day shift
• ***Approximately 2-3 evening home visits required (after school for children)***
• occasional Saturday shift
• Extensive field-based work, requiring case manager to travel to client homes throughout the region --
  
• Heavy case documentation
• Heavy travel to home visits
• Complete occasional ongoing training
• Must have reliable transportation
• No on-call
• Full-time, 40 hours per week
