Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) in Glenmoore, PA, to provide hands-on support to individuals with IDD in home and community settings. You will help with daily living, meals, mobility, and health monitoring, while documenting care and participating in team meetings.

This field-based role requires ensuring safety, dignity, and independence, with travel to homes and program sites and a flexible schedule including evenings or weekends as needed.

Qualifications

• High school diploma or equivalent.
• English proficiency required.
• Experience in caregiving or home health preferred.
• Ability to use electronic documentation and EVV systems.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal prep, mobility, and medication as authorized.
• Monitor health, safety, and emotional well-being; promote independence.
• Document accurately and timely; participate in trainings and team meetings.
• Travel to homes, community locations, and program sites as assigned; flexible schedule.
