Job summary

PALS OHIO, LLC. seeks an In-Home Care Specialist to provide hands-on support and supervision in home/community settings. Responsibilities include personal care, health management, meal planning, mobility and transportation, and skill-building activities tailored to each individual's plan.

The role requires teamwork, reliability, and the ability to follow individualized service plans while maintaining safety and compassion in daily activities.

Qualifications

• Minimum of High School diploma or GED.
• Current Ohio Driver's License with acceptable driving record.
• Criminal Background verification with acceptable clearance.
• At least one year of healthcare experience or working with individuals with disabilities.
• Ability to work as a team member and communicate positively with persons served and staff.

Responsibilities

• Safely provide transportation as needed.
• Ensure all safety equipment is in good working order.
• Responsible for individual supervision and support.
• Maintain documentation, including transportation, billing and program notes.
• Contributes to regular staff meetings.
• Promotes confidentiality, people-first language, empowerment and respectful treatment.
• Maintain current CPR, First Aid, Crisis Intervention, Behavior Support Plan and other required training.
• Other related duties as requested.
