PALS CHRYSALIS HEALTH CO. is seeking an In-Home Care Specialist in Heath, OH to provide hands-on support under an individualized service plan.

Responsibilities include homemaker and personal care, health and medical care, documentation, and transportation while maintaining safety and dignity for adults with disabilities. Required qualifications include a high school diploma or GED, valid OH driver’s license, and strong teamwork and communication skills.

Qualifications

• Requires High School diploma or GED.
• Current Ohio driver's license with acceptable driving record.

Responsibilities

• Safely provide transportation as needed.
• Insure all safety equipment is in good working order.
• Responsible for Individual supervision/support.
• Responsible for documentation, including transportation, billing and program documentation.
• Contributes to and attends regular staff meetings.
• Promotes a supportive environment by modeling company guidelines and people-first language.
