Job summary

Select Home Health Services, Inc. in Greenwood, IN seeks a skilled Certified Occupational Therapy Assistant (COTA) to provide in-home therapy under the supervision of a Registered OT.

You will deliver direct patient care based on the plan of care and monitor progress in patients’ homes in Greenwood, IN. Responsibilities include implementing therapy activities, training in self-care, documenting responses, and collaborating with the care team to ensure safe independence.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Indiana COTA license.
• Minimum of 1 year of experience in a hospital, facility, or home health setting; home health experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide in-home occupational therapy interventions under supervision of the Registered OT.
• Deliver therapeutic activities and self-care training to restore function and independence.
• Educate patients, families, and caregivers on exercises, transfers, and activities of daily living.
• Observe, document, and report patient responses and changes to the OT and care team.
• Maintain compliant documentation and participate in ongoing in-service education.
