Job summary

Suburban Home Health is seeking a dedicated Occupational Therapy Assistant (COTA) to provide direct patient care in home settings for adults and geriatric patients in central Indiana.

You will collaborate with the interdisciplinary team to implement care plans, assist OT with therapeutic exercises, and educate patients and caregivers on treatment plans and adaptive equipment to improve daily living activities.

Qualifications

• Graduate of an accredited occupational therapy assistant program.
• Licensed as a Certified Occupational Therapy Assistant in Indiana.
• Minimum of 1 year of home health experience preferred, but not required.
• Excellent written and oral communication and presentation skills.
• Valid driver’s license and automobile insurance with reliable transportation.

Responsibilities

• Collaborates with the interdisciplinary team to implement a care plan with the physician.
• Provides therapeutic care to restore function and daily living activities under the direction of the OTR.
• Assists in education and delivery of the exercise program developed by the OTR.
• Instructs patients and caregivers on treatment plans and proper use of adaptive equipment.
• Documents patients’ progress in EMR.
