Job summary

Select Home Health is seeking a skilled Certified Occupational Therapy Assistant (COTA) to provide in-home therapy services under the supervision of a Registered OT. The role focuses on delivering direct patient care based on the plan of care, monitoring progress, and supporting patients and families toward safe independence at home.

The ideal candidate will have an accredited COTA program graduate status and Indiana licensure, with minimum one year of related experience.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program (required).
• Current Certified Occupational Therapy Assistant (COTA) license in the state of Indiana (required).
• Minimum of 1 year of experience in a hospital, facility, or home health setting; home health experience preferred.

Responsibilities

• Provide occupational therapy interventions under supervision of the Registered OT.
• Deliver therapy activities and self-care training to restore function and independence.
• Educate patients and families on exercises, transfers, daily living activities.
• Document patient progress and changes in condition for OT and DON/DOR.
• Assist with order, fabricate, or recommend adaptive equipment as directed by OT.
• Participate in team conferences to ensure coordinated, high-quality care.
