Elara Caring is hiring a Certified Occupational Therapist Assistant in Indiana to provide licensed OT services in patients' homes. You will follow established care plans, monitor progress, report changes, and educate patients and caregivers on home therapy exercises.

The role requires a valid COTA in-state license, 1 year of clinical experience, and the ability to lift up to 50 pounds. A driver's license or reliable transportation is required for travel between homes.

Qualifikationen

• Graduate of a certified occupational therapy curriculum established by the Occupational Therapy Association.
• Current, unrestricted COTA certification in the state of work.
• Minimum one year of experience as a COTA in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Aufgaben

• Provide occupational therapy services by following the care plan developed by the Occupational Therapist.
• Visit patients regularly and monitor their progress during each visit.
• Report changes in a patient's condition to the Occupational Therapist, physician, and care team.
• Follow physician orders and the established plan of care.
• Teach patients and caregivers home therapy exercises and safe techniques.
• Work closely with the care team to provide coordinated, high-quality patient care.
• Complete accurate and timely documentation for each patient visit.
• Help reduce avoidable hospital and emergency room visits through proactive patient care.
• Stay up to date on healthcare regulations and agency policies.
• Work in a physically active role that may include lifting, transferring patients, and occasional irregular or on-call hours.
