VitalCaring is hiring a Certified Occupational Therapy Assistant (COTA) to deliver personalized, goal-driven care in patients’ homes. You will work under the supervision of a Licensed OT, focusing on activities of daily living and functional independence.

The role emphasizes one-on-one patient care, autonomy with team support, and a collaborative care model with nurses and leadership, ensuring meaningful impact on patients’ quality of life.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Active COTA license/certification in the state of practice.
• Home Health experience strongly preferred.
• Current CPR certification.
• Ability to work independently in a field-based environment.
• Strong clinical observation, communication, and time management skills.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Deliver personalized, goal-driven home health care under the direction of a Licensed Occupational Therapist.
• Implement treatment plans to improve daily living activities.
• Provide hands-on therapy focusing on strength, coordination, mobility, and self-care.
• Retrain patients in ADLs (dressing, bathing, meal prep, etc.).
• Assess and report patient progress, function, and home safety needs.
• Educate patients and caregivers on techniques, exercises, and safety strategies.
• Identify changes in condition and communicate with the OT and care team.
• Complete timely, accurate documentation in the EMR.
• Support a safe, effective, and coordinated plan of care.
