Job summary

Qualifications

• Bachelor’s degree in a mental health discipline.
• Certification in case management and in-home intervention within 6 months of employment.
• Must be at least 23 years old with a valid driver license and clean driving record.
• Knowledge of child/adolescent mental health.
• Ability to collaborate with therapists and staff.

Responsibilities

• Provide linkage to community resources.
• Arrange transportation as needed.
• Monitor clients for least restrictive placement.
• Support program therapist with outreach therapy.
• Document progress per AltaPointe policy.
• Participate in performance improvement activities.
