Job summary

AltaPointe Health seeks a dedicated case management professional to support homeless clients within our community outreach programs. The role focuses on assessing needs, developing service plans, and linking clients to essential community resources to promote stability and independent living.

The candidate will collaborate with treatment teams, document progress, and ensure access to services like housing referrals and transportation. Experience in HMIS and client engagement is preferred.

Qualifications

• Bachelor’s Degree in a mental health discipline is required
• Must be 23 years of age or older, have a valid Alabama driver’s license, and proof of current liability insurance

Responsibilities

• Assess consumer needs and set goals and ISP plans
• Provide linkage to community resources
• Maintain placement in least restrictive environment
• Monitor progress toward goals and advocate for resources
• Collaborate with treatment team and outside sources
• Make multiple contacts with consumers weekly
• Assist with intake, follow-up appointments, and transportation needs
• Document contacts in HMIS and Avatar per grant requirements
• Engage with area shelters and camps to support outreach
• Attend area breakfasts to build rapport with the target population
