Qualifications

• Bachelor's degree in a mental health discipline required.
• Certification of case management preferred/required.
• In-home intervention training by Alabama State Department of Mental Health within 6 months of employment.

Responsibilities

• Assess and reevaluate consumer needs using CANS.
• Provide linkage to community resources.
• Monitor placement in the least restrictive environment.
• Document progress and outcomes per AltaPointe policy.
• Assist program therapist and provide outreach therapy services when needed.
• Provide crisis intervention and management (24-hour availability).
