Lifecare Home Health is seeking a compassionate Home Health Aide to support patients in their homes, monitor health and daily activities, and collaborate with the care team to maintain independence and dignity. This role emphasizes patient-centered care and adherence to standards.

You will assist with ADLs, meals, grooming, and safety, while maintaining excellent communication with families and the multidisciplinary team. A valid HHA/CNA cert, CPR, and a driver’s license are required.

Qualifikationen

• High School diploma or equivalent required.
• Home Health Aide Certificate or equivalent certification.
• Active CPR certification required.
• 1+ year of experience as a Home Health Aide or CNA.
• Strong English language skills (written and verbal).

Aufgaben

• Monitor and record patient condition and plan of care.
• Assist with activities of daily living and personal care.
• Provide housekeeping, meals, and transportation support as needed.
• Observe safety and infection control guidelines and report changes to the care team.
• Educate and support family members on care needs and procedures.
• Maintain patient information in EMR and coordinate with Care Team.
