Job summary

A Place at Home is seeking a Home Health Aide to deliver compassionate, in-home care under RN supervision. You will assist clients with daily living activities, maintain safety, and document changes in health status while fostering trust and independence in a home-based setting.

The role requires a state-approved HHA certification, strong communication, reliability, and physical stamina. A valid driver’s license and transportation are expected to support visits across assigned client homes.

Qualifications

• Completion of state-approved Home Health Aide certification course.
• Strong communication skills, both written and verbal.
• Compassionate and empathetic with a strong desire to help others.
• Ability to work independently with patience and understanding.
• Physically able to perform duties including lifting and movement.
• Reliable transportation and a valid driver's license.

Responsibilities

• Providing ADL assistance (bathing, dressing, grooming, toileting).
• Assisting with meal planning and preparation, observing dietary restrictions.
• Assisting with medication reminders per care plan.
• Performing light housekeeping to maintain a clean living environment.
• Assisting with mobility and transfers, ensuring client safety.
• Providing companionship and building a supportive relationship with clients.
• Documenting changes in client condition and reporting to supervising nurse.
• Following HIPAA guidelines and protecting client PHI.
• Participating in ongoing training and professional development.
