A Place at Home is seeking a Home Health Aide to join our home-based care team. You will provide compassionate assistance with activities of daily living under the supervision of registered nurses and follow each client’s individualized care plan.

Responsibilities include helping with bathing, dressing, meal prep, medication reminders, light housekeeping, mobility support, and documenting client changes while maintaining HIPAA privacy. Must have HHA certification and a valid driver’s license.

Qualifications

• Must have completed a state-approved Home Health Aide certification course.
• Strong written and verbal communication skills.
• Compassionate and empathetic with a desire to help others.
• Ability to work independently with patience and understanding.
• Physically able to perform ADLs and lifting as needed.
• Reliable transportation and valid driver’s license.

Responsabilités

• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
• Plan and prepare meals, observing dietary restrictions.
• Provide medication reminders per care plan.
• Perform light housekeeping to maintain a clean home environment.
• Assist with mobility and transfers ensuring client safety.
• Offer companionship and support client well-being.
• Document changes to client condition and report to supervising nurse.
• Follow HIPAA and protect client PHI.
• Participate in ongoing training and development.
