Qualifications

• High School diploma or equivalent.
• Home Health Aide certificate from an accredited training program.
• Minimum of 1 year of HHA or CNA experience.
• Strong English language skills, written and verbal.
• Active CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Monitors and records patient condition per Plan of Care.
• Provides personal care and assistance with ADLs.
• Housekeeping, laundry, meals, and light errands for patients.
• Assists with bathing, dressing, and grooming as needed.
• Educates families on safe lifting and patient care techniques.
• Documents in EMR and notifies branch leadership of changes.
• Maintains safety and adheres to care policies and procedures.
