Job summary

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• At least 18 years of age.
• Ability to read, follow care plans, and document services provided.
• Current CPR certification.

Responsibilities

• Provide compassionate, hands-on care that supports patient comfort, safety, and independence.
• Assist patients with daily living activities (bathing, dressing, grooming, toileting).
• Support mobility, transfers, and basic exercises per care plan.
• Maintain a clean, safe home environment and assist with meals.
• Take and record vital signs as trained; assist with medication self-administration.
• Document care accurately in the EMR and participate in team meetings.
