Job summary

Symbii Afton Home Health & Hospice is seeking a Home Health Aide to provide personal care, mobility assistance, and caregiver support in patients’ homes. Duties include bathing, grooming, toileting, feeding, and light household tasks as outlined by the plan of care.

Qualifications

• CPR certification is required.
• Strong empathy and compassionate patient care.
• Ability to read and follow written care instructions.
• Ability to work with minimal supervision.
• Hold a valid driver's license and reliable vehicle.

Responsibilities

• Provide personal care such as baths, grooming, and hygiene.
• Assist with mobility, transfers, and range-of-motion exercises as directed.
• Support patient with toileting, dressing, feeding, and nutrition.
• Record vital signs when ordered and report changes to the nurse.
• Maintain clean, safe living areas and assist with light housekeeping.
• Assist with medication self-administration as directed by a nurse.
• Follow care plans and organization policies; document care provided.
