Upward Health in New York's Valley Stream area seeks a Care Specialist to deliver chronic care management primarily in patients' homes and via virtual visits. The role focuses on guiding enrollment, coordinating with providers, and promoting adherence to care plans to improve outcomes.

You will build relationships, navigate care pathways, and educate patients, families, and caregivers while traveling within the assigned territory; a valid driver's license and transportation are preferred.

Qualifications

• Minimum 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant).
• High school diploma or GED required.
• Valid driver's license, auto liability insurance, and reliable transportation to travel within the assigned territory.
• Experience in chronic care management or working with chronically ill/elderly patients.
• Technologically proficient with basic computer skills and EMR systems.
• Experience with motivational interviewing, trauma-informed care, and care coordination.
• Strong interpersonal communication and ability to engage patients and team members.
• Ability to prioritize tasks and work independently in unstructured environments.

Responsabilités

• Deliver chronic care management to high-complexity patients, primarily in patients' homes (90%) and virtually (10%).
• Outreach to patients to guide enrollment, arrange virtual appointments with providers, and ensure adherence to care plans.
• Build relationships, coordinate care, and educate patients to promote health and prevent hospitalizations.
