Qualifications

• Minimum 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant).
• High school diploma or GED is required.
• Valid driver’s license with reliable transportation for territory travel.
• Experience in chronic care management or with chronically ill/elderly patients.
• Proficient with basic computer skills and EMR systems.
• Experience with motivational interviewing and patient-centered care.
• Strong interpersonal communication to engage patients and team members.
• Ability to prioritize tasks and work independently in a non-structured environment.

Responsibilities

• Deliver chronic care management to high‑complexity patients, primarily in patients' homes (90%) with some virtual care (10%).
• Direct outreach to patients; guide enrollment and facilitate virtual appointments with providers.
• Ensure patients adhere to care plans and coordinate care across teams.
• Build relationships to promote health, prevent hospitalizations, and improve outcomes.
