Job summary

Upward Health, Inc. is seeking a Care Specialist to deliver chronic care management to high‑complexity patients, primarily in patients' homes (90%) with some virtual visits (10%).

You will engage with individuals, guide enrollment, and help coordinate care to improve outcomes. You will build relationships, educate patients, and support adherence to care plans, while coordinating with providers and families.

Qualifications

• Minimum two years in community health or related field.
• High school diploma or GED required.
• Valid driver’s license with reliable transportation.
