Job summary

Ashealthnet is seeking a Registered Nurse Case Manager to lead patient assessments and plan of care within a home health setting. The RN will coordinate multidisciplinary care, review medications, and ensure documentation meets Medicare and agency guidelines.

The RN will educate patients and families, supervise aides and LPNs, and participate in case conferences and on-call requirements as part of a dynamic care team.

Qualifications

• RN with current license in the state of employment.
• Experience delivering home health nursing care.
• Strong communication and interpersonal skills.

Responsibilities

• Oversees patient care from start to finish of episode.
• Update doctor orders in patient chart as needed.
• Notify physician of significant changes in condition.
• Provide skilled nursing care in patients' homes as ordered.
• Perform OASIS assessments to inform plan of care.
• Document all treatments, assessments and education per policy.
