Job summary

Team Select Home Care in the United States is seeking a qualified registered nurse to serve as Case Manager, overseeing patient care and coordinating with physicians and staff under state regulations.

The role includes developing care plans, performing initial and ongoing assessments, managing data collection (OASIS/PAR), conducting on-call duties, and ensuring accurate documentation and communication with families and healthcare providers.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• Minimum (1) year nursing experience required; prior home health experience preferred.

Responsibilities

• Develops, delivers, implements, revises, and evaluates individualized patient-centered care plans under supervision of a physician.
• Provides case management to an assigned group of patients and ancillary staff.
• Understands and submits required data collection sets (OASIS, PAR) and other documentation within required timeframes.
• Makes initial evaluation visits and reevaluates patient nursing needs per state/payor requirements.
• Coordinates care between patients, families, agency, and other healthcare providers.
