Job summary

Team Select Home Care in Houston, TX seeks an experienced registered nurse to develop, implement, and evaluate individualized patient-centered care plans under physician supervision. Qualifications

• RN license in the state of practice.
• Minimum 1 year nursing experience; home health experience preferred.
• Strong communication and documentation skills.
• Ability to organize and supervise ancillary disciplines as required.

Responsibilities

• Develops, implements, and evaluates patient care plans under physician supervision.
• Provides case management for assigned patients and staff.
• Completes required data collection sets (OASIS, PAR) within timeframes.
• Makes initial and ongoing patient evaluations; revises nursing needs as required.
• Advocates for patient welfare and coordinates care among families, agency, and providers.
