OneWell Health Care is seeking an In-Home Caregiver to provide non-medical, in-home support to individuals with IDD and autism. You will assist with ADLs, companionship, light housekeeping, and wellness monitoring, following individualized care plans under RN supervision.

This field-based role requires compassionate communication, reliable transportation, and accurate documentation of daily care logs. A High School Diploma or GED is required; prior caregiver experience is preferred; CPR, First

Qualifications

• High School Diploma or GED required.
• Prior experience as a caregiver, home health aide, or similar role preferred.
• Experience supporting elderly or individuals with disabilities is highly desirable.
• Demonstrated ability to follow care plans and written instructions.

Responsabilités

• Assist with bathing, grooming, dressing, toileting, mobility, and safe transfers.
• Provide medication reminders and non-clinical pillbox organization.
• Prepare meals based on dietary needs and encourage hydration/nutrition.
• Perform light housekeeping such as laundry and cleaning.
• Support companionship, activities, and community participation.
• Provide transportation or accompaniment to appointments/errands when required.
• Monitor and promptly report physical, behavioral, or emotional changes to the RN Case Manager or family.
