Job summary

OneWell Health Care in Columbia, SC seeks an In-Home Caregiver to provide non-medical support to individuals in private homes. The role focuses on ADLs, companionship, light tasks, and wellness monitoring under supervision, with emphasis on safety and dignity.

The caregiver will document care logs, follow care plans, and maintain HIPAA compliance while coordinating with an RN Case Manager and family. Flexible schedule in a field-based setting.

Qualifications

• High School Diploma or GED required.
• Experience as caregiver or home health aide preferred.
• Ability to document care activities electronically (mobile app or EMR).
• Ability to communicate effectively in English (written and verbal).

Responsibilities

• The In-Home Caregiver delivers direct support services in private home settings.
• Assists with bathing, grooming, dressing, toileting, mobility and transfers.
• Provides medication reminders and non-clinical pillbox organization.
• Prepares meals based on dietary needs and supports nutrition and hydration.
• Performs light housekeeping and supports companionship and community participation.
• Documents daily care logs and reports changes to the RN Case Manager or family.
• Follows individualized care plans and maintains confidentiality and HIPAA compliance.
