SYNERGY HomeCare of Toms River is seeking a Part-Time Registered Nurse (RN) to lead clinical assessments, care planning, and supervision for home care clients in New Jersey. The role emphasizes patient-centered care, compliance, and teamwork with caregivers and healthcare partners.

The ideal candidate holds an active NJ RN license, at least two years of clinical experience, and comfort with documentation and coordination tasks.

Qualificações

• Active New Jersey RN license required.
• Minimum 2+ years of clinical experience.
• Prior home care/home health experience preferred.
• Strong patient assessment and care planning skills.
• Knowledge of state healthcare regulations and compliance standards.
• Experience in caregiver training/coaching/supervision is a plus.
• Excellent documentation and communication skills.
• Ability to work independently and as part of a team.
• Reliable transportation required.

Responsabilidades

• Conduct initial client assessments and ongoing reassessments.
• Develop and update personalized Plans of Care.
• Perform caregiver supervisory visits to ensure quality and compliance.
• Provide skilled nursing visits as ordered when applicable.
• Provide caregiver education and skills training to ensure safe, high-quality care delivery.
• Ensure compliance with New Jersey state regulations and agency standards.
• Complete accurate and timely documentation.
• Support case management and care coordination.
• Collaborate with caregivers, families, and healthcare partners to ensure continuity of care.
• Contribute to building and maintaining positive relationships within the community.
