Job summary

S & S Home Health Care Agency is seeking a compassionate Part-Time Registered Nurse to join our home-based clinical team in New Jersey. You will provide skilled nursing visits, support admissions, and help with clinical documentation to ensure regulatory compliance.

As a key member of the care team, you will collaborate with DON and interdisciplinary staff to implement individualized care plans, educate patients and families, and maintain high standards of safety and quality in home health care.

Qualifications

• Active RN license in New Jersey.
• Minimum 1 year of nursing experience; home health preferred.
• Strong assessment, documentation, and organizational skills.

Responsibilities

• Perform in-home skilled nursing visits and assessments.
• Assist with patient admissions and care planning.
• Document care accurately and in line with policies and regulations.
• Monitor patient conditions and communicate changes to the DON and physician.
• Educate patients and caregivers on disease management and home care.
• Ensure infection control and patient safety during visits.
• Collaborate with health professionals to ensure continuity of care.
