Registered Nurse (RN)

Responsibilities
- Oversee and coordinate the delivery of quality care in accordance with each client's individualized care plan and needs.
- Conduct home visits with clients within designated geographic territories.
- Monitor clients' conditions and communicate any changes or concerns to the appropriate Clinical or Client Services Manager.
- Review, follow up on, and properly document physician orders and changes in the plan of care.
- Perform comprehensive client assessments and reassessments as necessary.
- Coordinate care among clients, caregivers, families, physicians, and other members of the healthcare team.
- Provide ongoing guidance and support to caregivers to ensure care is delivered in accordance with the client's care plan and agency standards.
- Monitor the quality and continuity of services provided to clients.
- Accurately document observations, interventions, assessments, follow-ups, and evaluations pertaining to client care and services provided.
- Identify client care needs and assist in developing appropriate solutions to ensure the highest level of service and client satisfaction.
Qualifications
- Graduation from an accredited and approved nursing program.
- Current and unrestricted Registered Nurse (RN) license in the State of New Jersey.
- Previous experience in home care, community-based healthcare, or a similar setting, with a strong understanding of the unique needs and responsibilities involved in providing care in the client's home.
- Strong clinical assessment, communication, organization, and care coordination skills.
- Ability to work independently, make sound clinical decisions, and effectively coordinate client care.
- Ability to communicate effectively with clients, families, caregivers, physicians, and members of the healthcare team.
