E & S Home Care Solutions, LLC in South Plainfield, NJ is seeking an experienced Registered Nurse to join our field-based home care team. The RN will perform in-home assessments, develop and update care plans, and supervise Home Health Aides, delivering quality, client-centered care in clients' homes.

This role offers flexible part-time and per diem scheduling, independent field work, and the chance to make a meaningful difference.

Qualifications

Conduct in-home nursing assessments and reassessments.
Develop, review, and update individualized plans of care.
Perform supervisory visits for Home Health Aides (HHAs).
Evaluate clients’ health conditions and identify changes in care needs.
Ensure compliance with New Jersey home care regulations and agency policies.
Complete nursing documentation accurately and within deadlines.
Communicate effectively with clients, families, caregivers, and office staff.
Provide guidance and education to HHAs regarding client care needs.
