Job summary

SYNERGY HomeCare of Toms River is seeking a Part-Time Registered Nurse (RN) to lead clinical activities, conduct client assessments, and develop plans of care for home-based seniors in the Toms River area.

The role emphasizes collaboration with caregivers, families, and healthcare partners, with flexible hours and opportunities for documentation work from home. NJ RN license and 2+ years of clinical experience are required.

Qualifications

• Active New Jersey RN License (required).
• Minimum 2+ years of clinical experience.
• Prior experience in home care or home health preferred.
• Strong patient assessment and care planning skills.
• Knowledge of state healthcare regulations and compliance standards.
• Experience in caregiver training, coaching, or supervision is a plus.
• Excellent documentation and communication skills.
• Ability to work independently and as part of a team.
• Reliable transportation required.

Responsibilities

• Conduct initial client assessments and ongoing reassessments.
• Develop and update personalized Plans of Care.
• Perform caregiver supervisory visits to ensure quality and compliance.
• Provide skilled nursing visits as ordered (when applicable).
• Provide caregiver education and skills training to ensure safe, high-quality care delivery.
• Ensure compliance with New Jersey state regulations and agency standards.
• Complete accurate and timely documentation.
• Support case management and care coordination.
• Collaborate with caregivers, families, and healthcare partners to ensure continuity of care.
• Contribute to building and maintaining positive relationships within the community.
