Job summary

SYNERGY HomeCare of Teaneck seeks a motivated Registered Nurse (RN) to join our team. You will deliver compassionate home-based nursing care, assess client needs, and help implement individualized care plans for clients in their homes.

This role requires an accredited nursing program diploma, an active NJ RN license, and the ability to work independently while coordinating with families and healthcare professionals to ensure high-quality, client-centered care.

Qualifications

• Graduation from an accredited and approved nursing program.
• Current and unrestricted RN license in the State of New Jersey.
• Experience in home care or community-based healthcare.
• Strong clinical assessment, communication, organization, and care coordination skills.
• Ability to work independently and coordinate client care.
• Effective communication with clients, families, caregivers, physicians, and healthcare team.
• Compassionate and dependable with commitment to client care.

Responsibilities

• Oversee and coordinate care per client plan.
• Conduct home visits within designated geographic territories.
• Monitor client conditions and report changes to the clinical team.
• Review physician orders and updates to the plan of care.
• Perform client assessments and reassessments as necessary.
• Coordinate care among clients, caregivers, families, physicians, and the healthcare team.
• Guide caregivers to follow care plans and agency standards.
• Monitor quality and continuity of services.
• Document observations, interventions, assessments, follow-ups, and evaluations.
• Identify client care needs and help develop solutions.
